Description

There are around 36M patients/year in urban India, who need post-acute supportive care away from hospital after discharge. They miss continuity of care from their specialist and access to supportive care from a familiar doctor. Such patients suffer from frequent episodes of symptoms that lead to re-admissions or re-visits to hospitals, when not managed in time. As a result, they suffer poor treatment outcomes, anxiety, and pain.

On the other hand, Specialists and Speciality hospitals want to free up capacity by optimising post-acute care duration in the hospital and expecting the patients to return for follow-up care. However, many patients do not return for follow-up care. And those who do, return to present poor patient outcomes due to the discontinuity of care and lack of specialised supportive care.

This is a double-sided problem that has not been addressed despite the growth of home healthcare and adoption of teleconsultation.

Impact

This problem impacts the patients and kin as well as the healthcare providers. A post-acute oncology patient, for example, typically suffers 2 to 3 episodes a month and more towards the end stage. He and his kin have to make repeated visits to the hospital in order to receive supportive or rehabilitative care. This also leads to increase in healthcare costs and loss of productivity of the kin. A secondary/ tertiary care hospital gets burdened by the discharged patients returning for supportive care or rectification procedures due to lack of compliance and poor care procedures. Apart from the economic loss the hospital deprives more needy acute care patients of specialised medical attention. The treatment outcomes are not controlled, which presents a healthcare efficacy problem. The primary healthcare infrastructure does not have the expertise to deliver such specialised follow-up care to the community. At a national level this deepens the gap in health and access to specialty care across the country.
